Home and Community Based Waiver Program

This is available in the Bronx and Rockland County.  It is an intensive program, funded by the Office of Mental Health that works with families in which a child has been hospitalized and is at high risk for the child to be hospitalized again. Services offered include: Individual care plan, tutoring/skill building, respite, In home therapy services, and Mental Health advocacy. In the Bronx, eligibility is determined by NYC C-SPOA and in Rockland County, eligibility is determined by SPOA in Rockland
