Blended Case Management Program

This program funded by the New York City Department of Health and Mental Hygiene is available in the Bronx for children who are high-end users of mental health services. 

There are two levels of service based on family stability and child’s mental health status. Services offered include:  In- home parent counseling, mental health services advocacy and monitoring, education advocacy and monitoring and wrap-around services.  Eligibility is determined by NYC C-SPOA.
