Dominic's Home is committed to providing caring, respectfully delivered
services to individuals with developmental disabilities and their
families in a personalized and professional manner. Children and adults
with a wide range of developmental disabilities are served through
our Intermediate Care Facilities, Individualized Residential Alternatives,
Day Habilitation Program, Service Coordination and Community In-Home
Services.
A therapeutic residential program for moderate to severe mentally
retarded adults and children. Four ICF's (INTERMEDIATE CARE FACILITY)
level of care, medical, clinical community inclusion services are
provided. Elite Day Habilitation program provides an individualized,
community-based person centered and person driven service for each
individual.
Blauvelt, New York
A
therapeutic residential program for mild to severe mentally retarded
children. Four ICF’s (INTERMEDIATE CARE FACILITY) level of
care, medical, community inclusion and clinical services are provided.
- Goshen, New York
Community based residential programs for developmentally disabled adults are homes designed to assist individuals to become more independent, productive, and integrated into their community.
Five IRA'S (INDIVIDUALIZED RESIDENTIAL ALTERNATIVES) are located
in various residential communities throughout Rockland County,
NY

IRA'S (INDIVIDUALIZED RESIDENTIAL ALTERNATIVES) are located in
the Northeast section of the Bronx, NY
All of these IRA'S are located close to schools, day programming, major shopping areas, recreational facilities and centers for leisure activities.
Community in-Home Services, Respite, Residential Habilitation and
Service Coordination is offered to individuals living at home. Service
Coordination is provided to help families access and monitor medical,
day programming, clinical, habilitation and respite services. - Bronx,
NY and Rockland County.
All of Saint Dominic's Home services for individuals with
developmental disabilities adhere to the philosophy
of "person-centered planning."
